LPDC Forms
In order to support National Staff Development Council and other professional goals, the LPDC provides various tools that are available for your use. Please click at the appropriate place to download and print these forms and templates.
Framework for Professional Development Activity - Table 1. This eight-page graphic is a valuable tool for staff members to use in examining the various paths for professional development. Of particular interest are Group C, District and/or Building Activities, and Group D, Individual Activities. Please note that since these two Groups are customized for individual use, they require pre-approval by the committee. Staff members need to complete the individual activity form to start the approval process.
Professional Growth Record for Licensure – Form E. The Form E must accompany your certification or licensure renewal materials. The applicant must complete this form and indicate whether Option 1, 2 or 3 is being exercised. If using Option 2 or 3, copies of all CEUs and official transcripts must accompany this form, along with the ODE certification or licensure renewal application. This form will also be used by new employees to establish a cumulative total of contact hours to start their enrollment in the MyLearningPlan system.
LPDC Verification Form – Five Year License - Form F. The Ohio Department of Education requires each LPDC to verify that an individual has on file an approved individual professional development plan, based on the needs of the “educator, the students, and the school district,” and thus verify that all requirements for a license renewal have been met. Each professional staff member needs to fill out Form F for completion by the LPDC.
Approval Verification for Educators Leaving a LPDC Ohio school districts and educational service centers will accept continuing education credit and university semester and quarter hours accumulated under an individual professional development plan approved by a LPDC. When an educator accepts employment in another school district or educational service center, the educator needs to provide documentation for the amount of continuing education credit accumulated since the issuance of a current license. The MLP system provides a transcript of all accrued credit and serves as the verification form of credit earned that can be used by an educator accepting employment with another Ohio school district or educational service center.
